The following are 14 questions that will help me to create a formula that will better suit your constitution.

We are asking questions about how you’re experiencing your bodymind so that we can identify patterns to help match you up with complimentary plants.

Do you feel like you tend to run hot or cold?

Are you full of opinions and like to share them, or mostly keep thoughts to yourself?

Are you usually thirsty, or do you rarely think about drinking water?

When do you feel most energetic – morning, afternoon, or evening?

Have you been feeling more anxious or down lately? If yes, for how long?

Stress Response:

Where do you hold tension when stressed?(shoulders, jaw, stomach, etc.)

After something stressful happens, how long until you feel settled – minutes, hours, or days?

Sleep & Digestion:

When you lie down to sleep, does your mind race or do you fall asleep easily when conditions are right?

Do you wake up feeling rested or still tired?

Do you often have loose stools, constipation, or something else?

Lifestyle & Health:

Do you mostly cook from fresh, from packaged foods, eat out, or some combination? Please describe

How many days a week do you move for 20 minutes or more?

Are you currently taking any medications (including those for anxiety, depression, or sleep)? Do you have any known allergies or health conditions?
